BMJ study ties GLP-1 kidney benefit to albuminuria status in type 2 diabetes

- 01In 75,455 US patients with type 2 diabetes followed about 32 months, five-year risk of kidney function decline with albuminuria was 3.2% on GLP-1 receptor agonists or SGLT-2 inhibitors versus 5.4% on DPP-4 inhibitors, a 40% lower risk.
- 02The findings suggest albuminuria status should factor into which agent is chosen after metformin, since 82% of the cohort had no albuminuria and showed little separation between GLP-1/SGLT-2 and DPP-4 arms.
- 03This is an observational target trial emulation of records and claims data from 2014 to 2022; the authors cite short follow-up and possible unmeasured confounding and urge cautious interpretation.
