The wire

  1. 01The recall covers 18 mg/mL compounded tirzepatide in 4 mL multidose vials, lots AP-202622 through AP-202625 and AP-202627 through AP-202644, plus one 200 mg/mL NAD+ vial, lot AP-202647, all for lack of sterility assurance.
  2. 02Pharmacists and telehealth prescribers sourcing from outsourcing facilities should check vials and records; the FDA cited serious deficiencies in the firm's sterile production in a December 2025 warning letter, though it has not linked those findings to this recall.
  3. 03No illnesses or injuries have been reported; the recall, begun Sept. 24, remains ongoing, and patients with fever, redness, swelling or worsening pain after injection are told to seek medical advice.
