BMJ target trial finds GLP-1 and SGLT-2 renal benefit only with albuminuria
Among 75,455 people with type 2 diabetes on metformin, the five-year risk ratio versus DPP-4 inhibitors was 0.60 in albuminuric patients.

- 01Target trial emulation across 10 health systems and two insurers estimated five-year renal deterioration risk of 3.2% on GLP-1 or SGLT-2 versus 5.4% on DPP-4 inhibitors in albuminuric patients.
- 02Among the 81.8% without albuminuria, the risk ratio was 1.10 (95% CI 0.90 to 1.38), suggesting little renal benefit over a median 32 months of follow-up.
- 03Observational design pools GLP-1 and SGLT-2 into one exposure group, so the contribution of each drug class to the albuminuria finding is not separated.
