GLP-1 users with MCI and diabetes show 26% lower dementia incidence

The wire
- 01In propensity-matched patients with type 2 diabetes and mild cognitive impairment, incident dementia occurred in 101 GLP-1 initiators versus 132 DPP4i initiators, hazard ratio 0.74 (95% CI 0.57-0.95).
- 02Cumulative incidence at 3.9 years was 15.5% versus 20.4%, an absolute difference of 4.9%, giving clinicians a real-world counterpoint to negative trials in early Alzheimer's disease.
- 03Authors call the work hypothesis-generating; the EVOKE and EVOKE+ randomized trials found oral semaglutide did not slow cognitive decline in early Alzheimer's, where 86% lacked diabetes.
