Meta-analysis finds GLP-1 benefit in HFpEF but not HFrEF

- 01In phenotype-based analysis, GLP-1 receptor agonists cut MACE (HR 0.79), heart failure hospitalization (HR 0.48), the composite of cardiovascular death or worsening heart failure (HR 0.57) and all-cause mortality (HR 0.75) in HFpEF.
- 02In HFrEF only cardiovascular death was reduced (HR 0.71), a split that matters when weighing incretin therapy in heart failure patients by ejection fraction.
- 03The analysis pooled dedicated trials with heart failure subgroups from cardiovascular outcome trials, used a pragmatic 40% ejection fraction threshold, and searched literature only to August 10, 2025.
