SELECT analysis finds semaglutide cut MACE 26% in patients at fibrosis risk
Fatty liver index fell 28% more with semaglutide than placebo over 104 weeks in the prespecified secondary analysis published in Nature.
The wire
- 01In a prespecified SELECT analysis, patients with Fibrosis-4 scores of 1.3 or higher had a 26% MACE reduction on once-weekly semaglutide (HR 0.74; 95% CI 0.63–0.88; P = 0.0004) over 104 weeks.
- 02The signal held across age-specific FIB-4 cutoffs, supporting use in obese patients with cardiovascular disease and suspected liver fibrosis without diabetes.
- 03The subgroup with FIB-4 above 2.67 showed a 34% reduction that was not statistically significant (HR 0.66; 95% CI 0.39–1.10; P = 0.11), and FIB-4 and fatty liver index are noninvasive surrogates, not biopsy.

In the SELECT trial, once-weekly subcutaneous semaglutide reduced major adverse cardiovascular events (MACE) by 20% versus placebo in patients with atherosclerotic cardiovascular disease and obesity but without diabetes. We examined semaglutide in SELECT patients at high risk for substantial liver fibrosis in a prespecified secondary analysis. Liver biochemical tests and steatosis risk according t
