GLP-1 users on dialysis show lower mortality than DPP-4 inhibitor users
A target trial emulation in the US Renal Data System also found bariatric surgery carried higher one-year mortality than GLP-1 therapy but more transplant waitlisting.

- 01Using 2010-2022 USRDS data, researchers emulated three trials in adults with end-stage kidney disease, type 2 diabetes and at least class II obesity: surgery versus GLP-1 (N=1818), surgery versus DPP-4i (N=2111) and GLP-1 versus DPP-4i (N=4338).
- 02Bariatric surgery carried higher short-term mortality than matched GLP-1 users (1-year aHR 2.68, 95% CI 1.98-3.63) but comparable long-term mortality, while GLP-1 users consistently outlived DPP-4i users.
- 03Surgery was associated with more kidney transplant waitlisting than either drug; this is observational registry analysis with residual confounding despite matching and active-comparator design.
