The wire
- 01Pooled randomized data through April 2026 showed no cognitive benefit (ADAS-Cog SMD -0.05, 95% CI -0.12 to 0.02) and no effect on progression (CDR-SB mean difference 0.09), with adverse events higher (RR 1.07).
- 02Clinicians fielding patient questions about incretins for dementia prevention now have randomized pooled evidence against a cognitive indication in established disease.
- 03Biomarker and neuroimaging results were inconsistent; the authors suggest biomarker-enriched, metabolically selected populations or more brain-penetrant agents as the remaining open question.
